Nearly every khenra is born a fraternal or identical
twin, and a pair of khenra twins forms an extremely
close emotional bond unknown to most other residents
of Amonkhet. The death of one twin in training or the
trials causes a tremendous shock to the survivor, who
typically grows more aggressive and foolhardy in battle.
The rare khenra who are born without twins are believed
to have killed their siblings in the womb, and are
thus viewed as natural-born initiates, sure to achieve a
glorified death in the Trial of Zeal.

Here’s what I’d do:

Khenra Twins. [positive bonus]. However, if your twin dies, or you were born without a twin, you are instead filled with a fury in battle that can not be quenched. You must succeed on a DC 15 Wisdom saving throw whenever you take damage in combat. On a failed save, you must attack the creature that damaged you until you drop to 0 hit points or it does, or until you can’t reach the creature to make a melee attack against it.
