If one of the Tea Lady’s living neighbors is evil, then her ability has no effect (because she does not have two living good neighbors) and neither neighbor is protected. Recall that Tea Lady’s ability only checks her living neighbors.
Abilities do not work post-mortem unless specifically stated to do so. This was actually covered in the OP, grumble at you for not reading OP.
There is a practical limit based on how many abilities are present that can possibly cause malfunction; since all abilities that can cause malfunction are made clear in the OP, you should be able to calculate that for yourself.
No, because making that information public would indicate how many characters that could cause malfunction are present. If any effects outside of character abilities that could cause malfunctions were present, however, they would be made known to the players.

Unless you mean to say that the information Grandmother and Godfather receives is scheduled to be given at the End of Night 1, then it is indeed impossible for them to malfunction at Night 1.
As for Lunatic… yeah, no. That one is impossible to malfunction.
In general nights are processed in this order:
- Characters who can make others malfunction
- Kills
- Information Gathering
and they all resolve immediately meaning if you’re killed you can’t gather information, and that malfunctions will apply to basically everyone
