I propose reducing feedback in general, but increasing it in particular for Nobles. For example:

Physicians are still informed when they heal an attack or cure a bleed, but not whether it was an attack or a bleed. Only a Noble’s Gossip can tell. 
- 
Butlers and Drunks are not informed whether their target was immune. Only cooperation with a Noble can determine this. 
-
