They do sometimes make unbalanced setups but we can always yell at her after game if they included more then one neutral in this setup.

And why would there have to be a town aligned healer or a healer at all. Celeste would be the perfect balance for an NK + scum game. You’d have to heal someone who did the kill unless you did it.
